Welcome to the Forms and Documents section. Here you will be able to access all the forms you may need for Sports and Societies. Simply click on a link for the form you need to access, and a DOC copy of the form will open in a new window.
You can complete the form electronically by clicking in each field and typing in the relevant information. Once you’ve completed the form, you can either select the PRINT option and return direct the Sports & Societies Office in the SOAP Centre, or you can click on the EMAIL option to send the form immediately to the Sports & Societies inbox. (Please note that some forms will only include an option to PRINT the form, as a signature is required).

Drop-in sessions at the Sports and Societies office take place every Tuesday 2-4pm and every Thursday 10am-12 noon.
Join Sports and Societies
You can sign up and pay membership fees for any of our Sports and Societies at the SOAP Centre, Brayford Campus. Also, you are able to sign up and pay membership fees online.
